Our Street is a street near you, somewhere in Yorkshire. The people on Our Street will help explain how current health and social care services work in your area, and how the proposed changes and improvements will benefit everyone by changing how hospitals and community services work.
Our Street is part of the communications and engagement around transforming health services across North Kirklees and Wakefield. It follows on from Meeting the Challenge, which was a consultation about local hospital services.
The work is not unique to this area. Many NHS organisations across the country have looked at how their services are provided and have made major changes to make them fit for the 21st century. It includes more ‘out of hospital’ care, as well as looking at how services can work together in a more joined up way for the benefit of patients.
Our Street has been developed by the local NHS, including North Kirklees and Wakefield clinical commissioning groups, Mid Yorkshire Hospitals NHS trust and other partner organisations. Patients, clinicians, patient representatives and partner organisations have been involved and will continue to be as the process continues.
